Every enterprise has AI agents touching live systems. Some were deployed by IT. Many were deployed by individual teams — a data analyst here, a developer there — armed with an API key and a Friday afternoon. The agents are multiplying. The governance is not.

This is a pattern that should feel familiar.


1 · A Pattern Everyone Should Recognise

Cast your mind back to 2018. Enterprise automation was having its moment. Every operations leader had a mandate to deploy bots, and every IT team was scrambling to make it happen. Workflows were spinning up fast — too fast. Within two years, most organisations had automation deployments they couldn't fully account for. Bots running in production that nobody owned. Processes that had quietly broken and self-repaired in ways nobody understood. Audit logs that didn't exist.

The technology had moved faster than the governance around it.

Now look at where we are with AI agents in 2026. The same pattern is repeating — but the stakes are materially higher. An unmanaged RPA bot stalls a process. An unmanaged AI agent can modify records, send communications, escalate decisions, and trigger transactions in ways that are genuinely hard to unwind.

"The technology is moving faster than the governance around it." We have heard this sentence before. The difference this time is that agents don't just store data — they take actions.

The most important question to answer before you scale your AI agent programme is not which model to use or which framework to build on. It is this:

Who is governing your agents?


2 · What Governing an AI Agent Actually Means

When most people talk about AI governance, they mean policy documents, ethics frameworks, and model cards. That work matters — but it is not operational governance.

Operational governance means the infrastructure sitting underneath your agents that answers the questions your compliance team, your security team, and your operations team will eventually ask.

Governing an AI agent means being able to answer five questions at any point in time:

  1. Who authorised this agent to access the systems it is touching?
  2. What did it decide — and what was the reasoning behind each action?
  3. If it made a mistake, where is the full audit trail?
  4. Which team owns it, and who is on call when it breaks?
  5. Can you stop it without breaking three other things that depend on it?

If you can answer all five clearly and immediately — with evidence, not assumptions — you have governance. If even one gives you pause, you have a gap. And gaps at the agent layer are a different risk category to gaps at the application layer, because agents are not passive. They act.

3 · The Four Governance Gaps Most Enterprises Have Right Now

These are not hypothetical risks. They are the gaps that appear consistently across enterprise AI deployments today.

No Identity Layer for Agents

Your human employees have identity. They authenticate, they have roles, they have access scoped to what they need. Your AI agents often do not. They run under shared service accounts with broadly scoped permissions, with no record of which agent used which credential to do what. When something goes wrong — and it will — this is where investigations stall.

No Execution Audit Trail

An agent that takes an action and produces no structured log of why it took that action is, from a compliance standpoint, indistinguishable from an arbitrary action. For regulated industries — finance, healthcare, insurance — regulators are beginning to ask specifically about AI-driven actions. "We don't have logs for that" is not an answer that holds.

No Approval Checkpoints

Not every AI agent action should require human approval. But some should — specifically the high-stakes, low-reversibility decisions. Most agents deployed today are binary: fully autonomous or fully supervised. There is no middle ground where the agent proceeds confidently on routine decisions and pauses on the edge cases that warrant human review. That middle ground is what mature agent infrastructure makes possible.

No Centralised Visibility Across Agents

As agent deployments multiply across teams and departments, nobody has a complete picture of what is running. Two agents might be modifying the same dataset with conflicting logic. An agent built by one team might be triggering downstream processes owned by another. Without a centralised control plane, these conflicts are invisible until they surface as incidents.

4 · Governed vs. Ungoverned: The Same Agent, Two Infrastructures

The difference between a governed and ungoverned agent is not in what the agent does — it is in the infrastructure around it. The agent itself can be identical. Everything that determines whether it is safe to operate at scale sits in the layer underneath.

5 · The Infrastructure Question Before You Scale

Building AI agents has become easy. The ecosystem is mature, the models are capable, the tooling is excellent. Any reasonably skilled developer can have an agent in production in a day.

Governing those agents at scale is not easy. It requires a control plane — infrastructure that most organisations have not yet built. Agent-building frameworks get you the agent. They do not get you:

  • The orchestration layer that manages execution across teams and environments
  • The identity and access management scoped to each agent's least-privilege needs
  • The structured audit trail that traces every decision back to its inputs and reasoning
  • The approval workflow engine that routes high-stakes decisions to human review
  • The centralised visibility across all agents running in your organisation

Building agents is now a solved problem. Governing them in production, at enterprise scale, is the open problem. That gap will define which AI programmes succeed and which spend two years untangling ungoverned deployments.

The organisations that build or adopt a proper agent control plane now will scale their AI programmes without the governance crisis that is already approaching for everyone else. The ones that wait will spend the next two years doing what operations teams spent 2020 doing for RPA — untangling deployments, rebuilding trust with compliance, and wishing they had built the foundation first.

The question is not whether you need agent governance infrastructure. It is whether you build it before or after your first serious incident.


6 · A 5-Question Self-Assessment

Use these to surface your governance gaps today. If you cannot answer any one of them confidently, that is the starting point.

  1. Can you list every AI agent running in your organisation right now — including ones built by teams outside IT?
  2. Do you have a structured audit log for agent decisions — one that shows not just what happened, but the reasoning behind it?
  3. Is every agent running under a scoped identity — not a shared admin account or a developer's personal API key?
  4. Do you have a human approval process for high-stakes agent decisions — or is every agent operating fully autonomously regardless of consequence?
  5. If an agent acted incorrectly right now, how long would it take to identify what it did, why it did it, and how to contain the impact?

If any of these exposed a gap — that gap is where your agent governance work begins.
